Prescription affordability: Assistance Options for Anticoagulants

There is help available for many people who can’t afford their anticoagulant (blood thinning) medication. Many pharmaceutical companies provide free or discounted medicines through prescription assistance programs, also called PAPs. Each program is administered by the pharmaceutical company and has its own qualifying criteria.

We are unaware of a savings program for medications which are not listed below.


Eliquis® (apixaban)

Bristol-Meyers-Squibb (BMS), the maker of Eliquis, offers a discount card as well as a free 30-day initial supply. Eligibility is determined on a case-by-case basis.

Free 30 day trial supply

The BMS Free Trial Offer allows you to receive the first 30 days of Eliquis free at participating pharmacies. You will need to ask your health care provider for an Eliquis “Free Trial Offer Card” at the time of your initial prescription which you will then need to activate.

Per BMS: “You may be eligible for the Free 30-Day Trial offer for Eliquis (apixaban) if:

  1. You have not previously filled a prescription for Eliquis;
  2. You have a valid 30-day prescription for Eliquis;
  3. You are being treated with Eliquis for an FDA-approved indication that an HCP has planned for more than 35 days of treatment;
  4. You are 18 years of age, or older; and
  5. You are a resident of the United States or Puerto Rico.”

Co-pay assistance

The Co-pay Card lets you pay as little as $10 per month for up to 24 months from activation, with an annual maximum benefit of $3,800.

Per BMS: “You may be eligible for the Co-pay Card for Eliquis® (apixaban) if:

  1. You are insured by commercial insurance and your prescription insurance coverage does not cover the full cost of your prescription, that is, you have a co-pay obligation for Eliquis;
  2. You do not have prescription insurance coverage through a state or federal healthcare program, including but not limited to Medicare Part D, Medicaid, Medigap, Veterans Affairs (VA), or Department of Defense (DOD) programs; patients who move from commercial plans to state or federal healthcare programs will no longer be eligible;
  3. You are 18 years of age or older; and
  4. You are a resident of the United States or Puerto Rico.”

Xarelto® (rivaroxaban)

Janssen, the maker of Xarelto, offers a prescription assistance program called Janssen CarePath.

Free 30-day supply

Eligible patients can receive a free 30-day supply of Xarelto® with a valid signed 30-day prescription for any Xarelto® dose other than 10 mg. The Trial Offer can be used with the Xarelto® Starter Pack. Refer to the Janssen CarePath Trial Offer brochure for full program requirements.

Patients can register to get a Trial Offer card via text message. Begin the registration process by texting “VOUCHER” to 29479.

Co-pay assistance

Per Janssen: “Eligible patients using commercial or private insurance can save on out-of-pocket costs for Xarelto®. Depending on the health insurance plan, savings may apply toward co-pay, co-insurance or deductible. Eligible commercial patients pay $10 per fill for XARELTO® (up to a 90-day fill), subject to program benefit limits. There is no limit to this benefit for the first 90 days, inclusive of the starter kit, and then a $200 limit for each 30-day supply thereafter. There is a $3,400 maximum program benefit per calendar year.

This program is not available to individuals who use any state or federal government-funded healthcare program to cover a portion of medication costs, such as Medicare, Medicaid, TRICARE, Department of Defense, or Veterans Administration.”

Hospital Discharge Offer

The Xarelto Hospital Discharge Offer provides eligible adult patients at risk for VTE post-discharge with up to 39-day supply of Xarelto® 10 mg at no cost. Pradaxa® (dabigatran)

Boehringer Ingelheim, the maker of Pradaxa, offers a prescription assistance program called SolutionsPlus.

Savings card

Per Beohringer Ingelheim, the eligibility requirement for a Pradaxa savings card:

“A) Commercially insured patients: 18 years or older may pay as little as $0/month, subject to a $2400 maximum annual program benefit or 12 uses, whichever comes first.

B) Government insured/cash-paying patients: 18 years or older whose prescriptions are paid for in part/full by state or federally funded program(s), like Medicare Part D, Medicaid, Veterans’ Affairs, Dept. of Def., or Tricare may receive 1 free 30-day supply. This offer is limited to one use per patient per lifetime.”

Savaysa® (edoxaban)

Daiichi Sankyo, the maker of Savaysa, offers a savings card.

Per Daiichi Sankyo, “For patients with commercial insurance, or patients without insurance, this savings card is applied after the following out-of-pocket expenses are met: $4 for a 30-day prescription or $12 for a 90-day prescription. This card is not insurance and does not cover deductibles. The maximum benefit is $270 per 30-day prescription or $810 per 90-day prescription. The card is not valid if enrolled in state or federally funded prescription benefit program (eg, Medicare Part D/Medicaid).

Lovenox® (enoxaparin sodium injection)

Sanofi Aventis offers a “Patient Connection Program” for Lovenox® (enoxaparin sodium injection) which provides the medication at no cost, based upon income eligibility. In general, you must have no insurance and have an annual household income of ≤400% of the current Federal Poverty Level. If you are enrolled in Medicare Part D, in addition to the criteria above, you must also spend at least 2% of your annual household income on prescription medications covered through your Part D plan in the current calendar year.

Fragmin® (dalteparin sodium injection)

Fragmin is available through Pfizer’s “Rx Pathways” program which Provides free medicines to eligible patients through their doctor’s office or, in some cases, at home. Per Pfizer, eligible patients must “have no prescription coverage, or not enough coverage, to pay for their Pfizer medicine, Reside in the U.S. or a U.S. territory Meet income guidelines, which vary by medicine, but start at 400% of the Federal Poverty Level, adjusted for family size.”
